From the Dom perspective, I don't remember a session that I had to completely change courses on. There is one session that stands out in my mind. A client came to see me when I moved to Rhode Island. He had followed my career from my Ohio days when I just started out. He was very excited to see me. But, when he left, I could tell he was disappointment. I reached out to him. He was polite and honest about what he felt was missing in the session. I asked him to send me an email describing how his fantasies, experiences, interests and how he imagined feeling in a session. He did. We got together again for a second attempt. It was fantastic. He was a weekly client until the time I retired. To this day we stay in touch and are close friends. This experience is what prompted me to stop using questionnaires and ask open ended questions about someone's feelings.
